Abstract

Introduction: skeletal craniofacial abnormalities without surgery or improper handling may result in important secondary deformity aesthetic and functional implications that should be considered for treatment.

Objective: to present a case in which a complication occurred within 35 years of the placement of silastic the orbital floor fracture of the zygomatic complex IV degree; associated with posttraumatic deformity.

Case report: a male patient aged 67 who reported having been treated to have redness of the eye and secretions. He reports that 35 years was operated by the presence of a fracture "in the left eye" and to put silastic, having no problems so far in this area. At physical examination facial facial asymmetry given by light enoftalmo the left eye, with pupillary gap, decreased eyelid opening, Antimongoloid trait, with erythema of the conjunctiva, chemosis and abundant secretions were detected. Súpero-limiting outward movement was found. The presence of a fistula in the pouch of the lower eyelid is confirmed. It is diagnosed as post-traumatic facial deformities complicated by the silastic reaction to a foreign body, for which he underwent surgery with removal of the material, performing reconstruction with titanium mesh and with excellent results.

Conclusions: traumatic injuries of the face are usually associated with high functional and aesthetic sequelae. Add to that the complications from the use of biomaterial implants, determining the therapeutic behavior can be challenging. Man posing of this case to transmit the experience to other familiar or colleagues with this rare clinical situation.
