Introduction: the exposed position of the ear makes it vulnerable to many types of injuries, including trauma.
Objective: present a case of reconstruction of the lost ear auricle due to trauma.
Case: a healthy 42 years male patient goes to University Hospital "General Calixto García" referring losing a piece of his ear in a fight. It was decided to perform hemostasis and fixation with stitches remaining pinna to the temporal region, after splitting in the area. At 3 months of the traumatic event it is re-evaluated for the purpose of performing reconstructive surgical planning. It is decided to perform a reconstruction by headset rib cartilage graft placed in a "pocket" in the temporal region. As of this last operation is performed under local anesthesia, the release of atrial region, the progress of a temporary placement flap and free skin graft on the back of the reconstructed area. Excellent postoperative evolution is observed. The patient reported great satisfaction with the aesthetic results obtained
Conclusions: in the clinical case presented a diagnostic and therapeutic sequence was used at the discretion of the surgeons involved, before a partial traumatic ear defect in its upper and middle thirds. This type of traumatic sequel required the use of surgical procedures in several operative times to guarantee a good blood supply of tissues and suitable final aesthetic results.
