Pénfigo: características clínicas, epidemiológicas y manejo terapéutico en un hospital de alta complejidad de Medellín, Colombia, 2019–2023

Resumen

Introducción: El pénfigo es una enfermedad ampollosa autoinmune poco frecuente, con amplio espectro clínico y necesidad de manejo multidisciplinario. En Colombia, los datos sobre su presentación y tratamiento son escasos, y la participación del equipo de salud oral en el diagnóstico temprano ha sido poco documentada.

 

Objetivo: Describir las características epidemiológicas y clínicas, así como las estrategias de manejo terapéutico empleadas en pacientes con diagnóstico de pénfigo atendidos en los servicios de Estomatología, Cirugía Maxilofacial y Dermatología de un hospital de alta complejidad en Medellín, Colombia.

 

Metodología: Estudio observacional, descriptivo y retrospectivo. Se incluyeron 51 pacientes con diagnóstico confirmado de pénfigo vulgar, foliáceo, herpetiforme u otros subtipos, atendidos entre enero de 2019 y diciembre de 2023. Se analizaron variables demográficas, subtipo clínico, gravedad y modalidades de tratamiento. El análisis estadístico incluyó frecuencias, medidas de tendencia central y desviación estándar, mediante IBM SPSS Statistics v.22.0.

 

Resultados: El 70,6 % de los pacientes fueron mujeres, con edad media de 41,12 ± 5 años. El pénfigo vulgar fue el subtipo más frecuente (78,43 %), con predominio de compromiso oral como manifestación inicial. El 78,43 % requirió terapia sistémica; la azatioprina fue el inmunomodulador más utilizado (66 %). Ningún paciente recibió rituximab por barreras de acceso.

 

Conclusiones: El pénfigo vulgar con compromiso oral predominante fue el subtipo más frecuente en este centro colombiano. El rol del equipo de salud oral en el diagnóstico temprano fue evidente. El enfoque multidisciplinario es indispensable para un manejo oportuno. Las barreras de acceso al rituximab, con implicaciones para la gestión sanitaria y las políticas de medicamentos, deben ser abordadas en futuras investigaciones
