Dental autotransplantation guided by a three-dimensional replica

Abstract

Introduction: Dental autotransplantation is the transfer of a donor tooth to a recipient site. It is indicated in cases of tooth loss due to caries, iatrogenesis or other causes. Currently we can use new technologies such as cone beam computed tomography and three-dimensional printers which can be used to make the treatment more predictable and effective.

Objective: To present the management of a patient’s dental autotransplantation using a three-dimensional replica of the donor tooth and the clinical outcome after one year of follow-up.

Case presentation: This is a 17-year-old male patient, with pulp necrosis and chronic apical abscess in the right maxillary first molar. Radiographically, a large radiolucent periapical image associated with the root apices was observed. Tooth extraction was indicated, but it was decided to perform a dental autotransplantation of the right maxillary third molar as a donor, using a three-dimensional replica for testing in the socket prior to extraction. At one-year follow-up, first-quadrant cone beam computed tomography showed complete root formation and a periodontal ligament space and pulp chamber with the presence of calcified tissue. Clinically, the patient was asymptomatic and no significant changes were observed.

Conclusions: The use of a three-dimensional replica of the donor tooth in the dental autotransplantation performed in this patient proved to be an effective and predictable approach, reducing procedural errors and operative times, with satisfactory clinical results after one year of follow-up.
