Therapeutical features of the facial cutaneous fistula

A cross-sectional study was carried out along 5 years (1984-1988), in which 99 patients with facial cutaneous fistulas were treated in the maxillofacial surgery service of the "Saturnino Lora" Educational Hospital, at Santiago de Cuba. It was confirmed that in order to establish favorable therapeutical prognosis in this entity, the treatment of the acute fistulas must be oriented towards eradicating or minimizing the acute phase by antimicrobial drug therapy of the kind of beta-lactam antibiotics, combined with the elimination of causal factors, followed in a mediate phase by fistulaplasty, in accordance with the individual requirements of each lesion. As for the chronic course lesions, the conduct must be directed towards the elimination of the causal agent, and exeresis of the fistulous section with a plasty of the affected area in the same surgical time, combined with prophylactic antibiotic therapy according to the bacteriological results. It is concluded that the postoperative evolution of this affection is significantly satisfactory, and the affections with an osteomyelitic origin have the highest complications incidence.Downloads
